Are you a vendor? Food Truck? Artisan? Crafter? Local business? If you are interested in becoming a vendor, please fill out the vendor application form below. Include your top 3 location choices and a photo of your booth. Payment is due in full by July 31, 2019, A copy of your Business Insurance and a City of Fruita Business License are required. Food vendors, please see the link below for additional requirements.

VENDOR APPLICATION: 2019 VENDOR APPLICATION – PDF File — Please emailed or mailed your complete application back to the Fruita Area Chamber of Commerce.

Application instructions: Save the below Vendor Application form to your computer, fill out the application, re-save a copy to your computer, then when you click the “Send via Email” button at the top it should open your email with the completed form attached.  The other option is after saving the Vendor Application form to your computer, complete the form, then click the “Print Form” button at the top and the form will be sent to your printer, then it can be mailed to the address noted on the form.  If you have any problems or concerns, please contact the Fruita Area Chamber of Commerce at (970) 858-3894.
